  1. Step 1

    Schedule before your card expires

    Plan your renewal evaluation well before your current Utah medical cannabis card expires. Starting early prevents a gap in your certification and gives you time to complete the state renewal process.

  2. Step 2

    Attend a telehealth renewal visit

    Your renewal is a convenient video appointment with a provider. You'll review how you've been doing, discuss any changes, and the provider will determine whether continued certification is appropriate.

  3. Step 3

    Get state application guidance

    After your renewal appointment, we guide you through the Utah state application steps so your renewal paperwork is completed accurately and submitted to the right state portal.

  4. Step 4

    Receive renewal reminders

    Renewal reminders help you stay ahead of your next expiration date. Together, state application guidance and reminders make the process easier to complete on time, year after year.

Initial evaluation ($130)

  • Government-issued photo ID
  • Proof of Utah residency when required
  • List of current medications and dosages
  • Relevant medical records or diagnoses
  • Previous provider notes, if applicable
  • A list of questions or concerns for the provider
  • Payment method for the $130 evaluation fee

Renewal ($85)

  • Government-issued photo ID
  • Current Utah medical cannabis card or patient number, if available
  • List of current medications and dosages
  • Updated medical records since your last certification
  • Notes on how you've been doing and any concerns
  • Payment method for the $85 renewal fee

For telehealth appointments

  • A quiet, private space with good lighting
  • A reliable internet connection
  • A smartphone, tablet, or computer with a working camera and microphone
  • Headphones for privacy, if preferred
  • The video-call link or app ready before your appointment time
  • Your ID and documents nearby in case the provider needs to review them
