    Choose an initial medical cannabis evaluation or renewal appointment.

    Complete Your Information

    Complete the required intake information and provide any requested documentation before your appointment.

    Meet With Your Provider

    Meet with a qualified healthcare provider to review your history, symptoms, previous treatments, and relevant information.

    Determine Eligibility

    The provider will determine whether certification is clinically appropriate and whether applicable Utah requirements are met.

    Complete Required State Steps

    If certification is appropriate, you will receive clear instructions regarding any additional steps you must complete through Utah's medical cannabis program. The state — not our practice — issues the medical cannabis card.

An important note about the state card

Balanced Clarity Mental Health performs evaluations and, when clinically appropriate, completes provider certification requirements. The Utah medical cannabis card itself is issued through Utah's state medical cannabis program, and patients must complete the state's own steps and requirements. Requirements may change, so please review current official state guidance.
